00:00:03,075 --> 00:00:08,034
Okay, now let's talk about one rescuer CPR with an AED.
2
00:00:08,034 --> 00:00:13,004
First things first, before we even consider using an AED, we need to be sure our environment
3
00:00:13,004 --> 00:00:16,027
is in such a state that we can use an AED.
4
00:00:16,027 --> 00:00:21,060
In other words, there are no combustible gases or liquids and no standing water that
5
00:00:21,060 --> 00:00:23,086
the body of the patient is laying in.
6
00:00:23,086 --> 00:00:27,056
If indeed we are by combustibles or we are in standing water
7
00:00:27,056 --> 00:00:30,095
we would move the victim to a place of safety, dry them off
8
00:00:30,095 --> 00:00:34,002
so that we could literally use the AED on them.
9
00:00:34,002 --> 00:00:37,018
This is specifically important around drowning patients.
10
00:00:37,018 --> 00:00:42,009
The body doesn't have to be completely dry, it's just that we have to make sure we're not kneeling
11
00:00:42,009 --> 00:00:45,016
in a pool of water that the body of the patient is touching
12
00:00:45,016 --> 00:00:48,054
we might suffer electrocution through that or have a shock.
13
00:00:48,054 --> 00:00:51,037
So, we're in a safe environment, the scene is safe, gloves are on,
14
00:00:51,037 --> 00:00:55,039
our CPR shield is available. We start the same way.
15
00:00:55,039 --> 00:00:58,067
Sir, are you okay? There's no response.
16
00:00:58,067 --> 00:01:02,029
He does not look like he is breathing. I am immediately going call
17
00:01:02,029 --> 00:01:03,081
for emergency medical services.
18
00:01:03,081 --> 00:01:08,087
You in the plaid shirt, go call 911 and return, I might need your help.
19
00:01:08,087 --> 00:01:11,049
Now that emergency medical services or a code has been called
20
00:01:11,049 --> 00:01:13,095
we're going to go right into a pulse check.
21
00:01:15,026 --> 00:01:20,043
I check for a carated pulse for no more than 10 seconds, I do not feel a pulse.
22
00:01:20,043 --> 00:01:26,012
The patient is not breathing. Now we are going to move right to the AED.
23
00:01:26,012 --> 00:01:27,067
Turn it on.
24
00:01:27,067 --> 00:01:32,014
AED prompt- [This is a training device only, no shock will be delivered]
25
00:01:32,014 --> 00:01:35,036
I like to take the pads out and the scissors.
26
00:01:36,015 --> 00:01:39,022
AED prompt- [Remove clothing from person's chest]
27
00:01:39,022 --> 00:01:43,081
If it wasn't already cut we'd have to cut it, or rip it open.
28
00:01:43,081 --> 00:01:47,063
AED prompt- [Attach pads firmly to person's chest, as shown]
29
00:01:47,063 --> 00:01:51,049
There are diagrams on the pads, it's a nice idea to have a towel
30
00:01:51,049 --> 00:01:59,045
to wipe off extra sweat, remove nitro glycerin pads or nicotine patches.
31
00:01:59,045 --> 00:02:01,086
AED prompt- [Check connector and pads]
32
00:02:01,086 --> 00:02:06,023
Keep in mind that the pads will show you right on them with a diagram
33
00:02:06,023 --> 00:02:10,087
as to where they go on the chest of the patient, so it's pretty simple.
34
00:02:10,087 --> 00:02:16,000
This one is going to go up here on the right top pectoralis region.
35
00:02:16,000 --> 00:02:20,046
The second goes mid-axillary right underneath the arm pit on the left side.
36
00:02:20,046 --> 00:02:23,003
Make sure the pads are sealed well.
37
00:02:23,003 --> 00:02:28,051
The connector only fits one way on most AEDs.
38
00:02:28,051 --> 00:02:32,079
AED prompt- [Analyzing rhythm, everyone stand clear]
39
00:02:32,079 --> 00:02:35,080
I'm not touching the patient, I'm not kneeling on the patient.
40
00:02:37,066 --> 00:02:43,022
AED prompt- [Shock advised, charging, everyone stand clear]
41
00:02:43,022 --> 00:02:44,030
Make sure everyone's clear.
42
00:02:44,030 --> 00:02:46,096
AED prompt- [Long Beep]
43
00:02:46,096 --> 00:02:50,077
AED prompt- [Shock delivered, begin 5 cycles of CPR]
44
00:02:50,077 --> 00:02:54,074
Now I'm just going to do what the prompt tells me to do on the AED.
45
00:02:54,074 --> 00:02:59,043
Start my 30 compression, 2 inches deep or more, at least 100 a minute.
46
00:02:59,043 --> 00:03:03,025
And then give 2 rescue breaths with the head tilt chin lift
47
00:03:03,025 --> 00:03:08,010
Using my one way valve with the mask and then 30 more compressions.
48
00:03:08,010 --> 00:03:13,090
Doing this, 30 to 2. Until the AED interrupts me and then it will
49
00:03:13,090 --> 00:03:16,010
tell me whether it is going to shock again.
50
00:03:16,010 --> 00:03:18,062
I simply follow the prompts as we continue this rescue
51
00:03:18,062 --> 00:03:21,050
and wait for emergency response to arrive.
52
00:03:21,050 --> 00:03:24,052
AED prompt- [One minute and thirty seconds of CPR remain]
53
00:03:24,052 --> 00:03:28,005
If for any reason the pads won't stick to the chest any more
54
00:03:28,005 --> 00:03:31,086
you can remove them and try a new set of pads.
55
00:03:31,086 --> 00:03:37,006
However, if there working. We do not disconnect the AED until
56
00:03:37,006 --> 00:03:40,095
advanced life support or the next level of care comes and takes over.
57
00:03:40,095 --> 00:03:45,038
That's an important piece to remember as we continue this rescue episode.
58
00:03:45,038 --> 00:03:48,020
[Begin simulation]
59
00:03:48,020 --> 00:03:52,070
Sir, sir are you okay? The scene is safe, my gloves are on, my shield is available.
60
00:03:52,070 --> 00:03:57,047
Sir can you hear me? There's no response and he doesn't seem to be breathing or breathing normally.
61
00:03:57,047 --> 00:04:02,009
You in the plaid shirt go call 911, please come back and let me know that they're on the way
62
00:04:08,051 --> 00:04:11,090
I don't easily feel a pulse.
63
00:04:12,085 --> 00:04:18,045
AED prompt- [This is a training device only, no shock will be delivered]
64
00:04:22,006 --> 00:04:25,093
AED prompt- [Remove clothing from person's chest]
65
00:04:30,013 --> 00:04:34,022
AED prompt- [Attach pads firmly to person's chest, as shown]
66
00:04:41,042 --> 00:04:44,010
AED prompt- [Plug in connector]
67
00:04:45,065 --> 00:04:48,033
AED prompt- [Check connector and pads]
68
00:04:48,033 --> 00:04:52,035
AED prompt- [Analyzing rhythm, everyone stand clear]
69
00:04:56,049 --> 00:05:00,048
AED prompt- [Shock advised.... Charging]
70
00:05:00,048 --> 00:05:02,050
AED prompt- [Everyone stand clear]
71
00:05:02,050 --> 00:05:06,023
AED prompt- [Long Beep]
72
00:05:06,023 --> 00:05:10,039

Make sure everyone's clear.

Adult AED Training Summary:
If the patient is a witnessed cardiac arrest, first check to assure the scene is safe. Check for patient responsiveness and contact emergency services. Check for a pulse for no more than 10 seconds. Turn on the AED if the patient has no pulse and is not breathing. Attach the AED pads to the patient, and do not touch the patient while the AED analyzes. After a shock is delivered, begin CPR for about 5 cycles or two minutes. The AED will interrupt after two minutes and reanalyze the patient. Continue to follow the AED's instructions until advanced life support arrives.
